I lost or misplaced my stock certificate. How do I request a replacement?
The fastest and easiest way is to register for a Publix Stockholder Online account. Then log in to Publix Stockholder Online, click View Certificates next to the account for which you’re missing a certificate and select Replace to complete and print the Affidavit of Lost Certificate and Replacement Request Form. Be sure to follow the instructions on the form.

Can I request a paper certificate?
No. Publix stock is only issued as an electronic certificate. Publix no longer issues paper certificates.
Can I exchange my paper certificate for an electronic certificate?
Yes. Stockholders are encouraged to exchange their paper stock certificates for the convenience and ease of electronic certificates. Simply register for a Publix Stockholder Online account. Then log in to Publix Stockholder Online > Account Tools > Replace Certificates and follow the on-screen process to complete and print the Replacing Publix Paper Stock Certificates for Electronic Form. Be sure to follow the instructions on the form.
Is there a fee for exchanging paper certificates for electronic certificates?
There is no fee for exchanging paper certificates for electronic certificates. If you’ve lost a paper certificate, a fee is charged based on the value of the stock certificate being replaced.
How do I find my certificate number if I don’t have a paper certificate?
You can find the certificate number, cost basis and other account information by registering for and logging in to Publix Stockholder Online.
